Final report

As part of passive monitoring, close out report updates the IRB on the conduct and outcomes of the study, including any risks that may have arisen since the last continuing review.

Investigator/s should use the IRB study report form to submit the close out report of their study for IRB review and approval. Final report shall be submitted to the IRB when investigators complete all the following activities of the study as applicable:

• Local recruitment
• Participants follow-up
• Data analysis
• Major findings
• Maintenance of biological repository in an un-identifiable information.
